Step two is to apply for your Colorado PA license through the Department of Regulatory Agencies (DORA): https://apps.colorado.gov/dora/licensing/Default.aspx

You will need to establish a new account to submit a new licensure application.

Graduation from an accredited PA program - submit this form

Verified practice history - submit this form
Not applicable to newly graduated PAs

NCCPA certification - submit this form

Verification of Federation of State Medical Boards disciplinary action - submit this form

Please note, Colorado does not issue temporary licenses. The Colorado Medical Board meets quarterly and recommend applying for a new Colorado license as soon as possible.

You will notice that there is a requirement to submit a Primary Physician Supervisor Registration Form to complete your licensure application. Note that you can complete the remainder of the requirements and then submit this form once you have a physician supervisory relationship established.
